Yes, definitely a Malus and possibly a crab apple; couldn't possibly suggest which cultivar.

The disease might be apple scab, caused by the fungus Venturia inaequalis.

Some info and guidance here, but it does not show the symptoms on the leaves

That also does not mention that the afected leaves often show a puckering, too - as yours do.

    More cultural advice here

Leaf photo (but note there is a good deal of variation in its appearance):

Despite the variability of the symptoms on leaves, something's niggling me; perhaps its partly the fact that I can see no sign of it on the (few) fruits I can see. There's a chance it could be something else.

...But, having thought and researched a bit (rusts, leaf spots, ertc.), I can't find anyhting closer. So apple scab is the best I can come up with.

Now I've given you that pointer, perhaps you can compare photos with more leaves (and fruit) on your tree. You might be able to decide it is or isn't.

If you are near an RHS garden, you can probably take a sample there for an opinion.

PS. wind can cause somewhat similar damage to leaves, especially if they flap about against other twigs
